HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is calculated from fasting glucose × fasting insulin ÷ 405. Values above 2.0 suggest insulin resistance. Above 2.9 indicates significant resistance.
Fasting glucose stays normal until very late in the disease process. Insulin rises YEARS before glucose does. HOMA-IR catches the problem 5-10 years earlier than glucose alone.
